Your Google Form will also include information to help you make your 2025benefits decisions, such as District HSA contribution amounts. You can view the form without submitting or editing your response once complete until the open enrollment window is closed.

2025 Voluntary Supplemental Health Plan Benefit Premiums:
There are a few noteworthy changes for 2025:
- New for 2025 the District will be offering Voluntary Supplemental Health Benefits
- Accident
- Critical Illness
- Hospital Indemnity
- As Voluntary Plans, employees would pay 100% of the cost for these benefits..
- The deductible and maximum out of pocket limits have increased for the CHI and NHN Narrow Network Plans to $3,300 (individual) / $6,600 (family).
- The Copays for the Traditional PPO Plan are changing as follows:
- Increase Rx Copays on the PPO plan
- Generics: $40
- Brand Name:$100
- Brand Name (non-formulary): $150
- Specialty: $250
- Increase Diabetic SupplyCopays on the PPO plan
- Retail - $25 / $110
- Mail Order - $62.50 / $275
- Increase Rx Copays on the PPO plan
- Adding CVS Pharmacy “True Accum” Program to the Broad Network HDHP, CHI HDHP, and NHN HDHP Plans
- With True Accum a member’s out of pocket costs are automatically adjusted so only true member cost share is applied toward the deductible or annual out of pocket maximum.
- CVS Virtual Care will be replacing Teladoc
- New Member ID Cards
- Because of the change to CVS Virtual Care, new ID cards will be issued. Member ID and Plan Information will remain the same.
